Online Psychologist: Does It Actually Work? Science Answers

“Real therapy only works face-to-face.” That is probably the most common myth surrounding online consultations. But what does science say? Spoiler: it disagrees with you.

When the whole world suddenly shifted online in 2020, psychologists and clients alike faced the same question: will this actually work just as well? Science gave an unambiguous answer — and it surprised the skeptics.

What the Research Says

In 2022, the journal World Psychiatry published a large-scale meta-analysis combining the results of 17 randomized controlled trials (more than 1,700 participants). The conclusion: online cognitive-behavioral therapy is equally as effective as in-person therapy for treating anxiety disorders and depression. The effect size was 0.83, which is considered “large” in psychological research.

A separate 2023 study by the American Psychological Association (APA) compared client satisfaction across both formats. Online scored 4.3/5 versus 4.1/5 for in-person — clients valued schedule flexibility and not having to travel to the office.

A study by Sucala et al. (PLOS ONE, 2012) examined precisely this question and found: therapeutic alliance ratings in the online format do not differ significantly from in-person. Clients establish trust, feel supported, and open up — regardless of whether they are sitting in an office or relaxing at home in a comfortable chair.
